This study determined the efficiency of FINDRISC tool embedded in a smartphone application in detecting and referring individuals to RHU as high risk for developing Diabetes and Hypertension. It utilized a cross-sectional design, with features quantitative and qualitative research, in which 239 Individuals more than 20 year old from Josefina ZDS were assessed by 13 BHWs for their risk of having Diabetes and elevated blood pressure. The completion rate when using smart-phone application for case detection was 82.42%, with 25.90% error rate. The mean duration of interview is at 04:56±2:43 minutes, with 10.04 % of the interviews had form of assistance sought which focus more on preventing errors. There were no errors found in the entry fields required for FINDRISC scoring and blood pressure entry. The method had efficiently detected high- risk and very high risk individuals for developing diabetes of which has a prevalence rate of 6.28% and 20.92% individuals with elevated blood pressure during assessment. Around 61.11% of detected individuals have been successfully referred to RHU, with an average referral time of 1.5 days. The successful urgent care referrals are considered higher compared to other studies in terms of proportion and duration. Various themes described the experiences of the BHWs in using smart-phone for case detection and referral. FGDs revealed that smart-phone had made case detection and referral very easy because of several features like automatic computation of risk scores, risk classification and provided appropriate advice for referral. Thus, it is highly recommended to utilize this method for case detection and referral of high risk individuals for developing Diabetes and elevated blood pressure.
